What is the Casper Test?

Casper measures your interpersonal and intrapersonal skills, professionalism, ethics, empathy, and bedside manner—outside of academics. It is one of three components within the Altus Suite, a multi-part assessment designed to evaluate a prospective medical student's ethics and social skills.

  • Casper is a 100-120 minute situational judgment test.
  • Snapshot is a short, one-way video interview.
  • Duet is a value-alignment assessment.

The Casper portion of Altus Suite is the most difficult, and it requires the most preparation. Casper stands for "Computer-Based Assessment for Sampling Personal Characteristics." The Casper test aims to unveil who you are beyond your grades and hard skills. It is scored based on the following 10 Constructs:

  • Collaboration
  • Communication
  • Empathy
  • Equity
  • Ethics

What to Know About Scheduling Your Casper Test

You may or may not need to take the Casper test as part of your medical school application. Whether or not you need to take the test depends on each of the schools you are applying to. Take a look at this list of every US medical school that requires Casper, but also check the Altus Suite website directly for the most recent changes.

You only need to take the Casper test once, and you are only able to take it once per application cycle. If you are unhappy with your performance, you cannot take Casper again, and if you reapply to medical school next year, you will need to retake the Casper test.

Keep in mind that Casper is only one part of the Altus Suite. You may also be required to complete a short Snapshot interview and Duet assessment depending on the requirements of each specific school.

New Casper test dates are released each year, generally in the spring, which is when the last possible dates from the previous application cycle conclude. You must sign up for a test date that matches the possible dates for the specific schools you're applying to.

If possible, schedule your Casper test sooner than later so that you're not scrambling to complete the test last minute or during a packed interview season. Many students find they have a few weeks of breathing room after primary applications are submitted but before secondaries roll out. If you can make it work, utilize this time to complete your Casper test as well as the other required Altus Suite components.

You are not able to reserve a Casper test until you create an Altus Suite account. After you create your account, you can follow these steps from Altus to reserve your preferred test date.

  1. Click '+ Reserve new test' on the Altus Suite homepage.
  2. Select the country which the program(s) you are applying to reside in.
  3. Select the language you would like to use when taking the Casper test, if applicable.
  4. Select the admission cycle for your program(s).
  5. Select the program type(s) you are applying to.
  6. Select the school(s) you are applying to (more can be added later).
  7. If prompted, type the application/student ID number(s) into the text field(s).
  8. Select your desired test date.
  9. Review your test reservation details.
  10. Input your payment information.
  11. Confirm and submit your test reservation payment.

Ensure you register for your test at least three days before you plan to take the test, as identity verification and payment processing can take time. Keep in mind that unless otherwise indicated, all test times are in EDT/EST. Double-check the date and time of your test to ensure you account for any time zone differences.

Preparing for the Casper Test

1 | Practice Within the Test Format

The Casper test is unlike any other test you've taken. It has a very distinct format that must be prepared for in advance. The test is composed of 15 scenarios, some video-based and some word-based. Each scenario is followed by three open-ended questions.

The Casper test is broken into two sections—a typed response section and a video response section.

For the typed response section, you must type answers to all three questions within a five-minute time frame. The typed response section is made up of three word-based scenarios and six video-based scenarios. You are automatically advanced to the next scenario once your five minutes are up.

For the video response section, you must record a video of yourself verbally responding to three questions, which come one after another. The video response section is made up of two word-based scenarios and four video-based scenarios. You are allotted one minute to record a video response to each question. Once your one minute is up, your response is automatically uploaded. You are not able to change your answer or view your response.

Having a clear understanding of the Casper format is key to success on the test. It's not enough to practice answering Casper test questions; you must practice within the Casper format. This way, you can get used to typing responses to three questions within five minutes and verbally responding to a question within one minute.

How much time you need to prepare depends on how comfortable you are answering practice questions effectively within the allotted time. Get started with practice questions early so that you have time for additional preparation or tutoring if you find you struggle with the time limitations. Casper offers a practice test that becomes available when you access your Altus Suite account.

2 | Practice If/Then Statements

Casper asks you tough, often ethically-based questions that don't have a clear right or wrong answer. It's important to show that you are able to quickly and effectively assess all aspects of the situation as well as the wide-ranging repercussions of your actions.

If/Then statements illustrate your ability to think ahead and consider possible consequences, and they allow you to add more context to the purposely vague scenario.

For example,

If a prompt describes a scenario in which you see one of your classmates stealing a textbook, you can use If/Then statements like this:

"I would confront the classmate and ask them if they need help paying for their textbooks.

If they agree to return the textbook, then I will offer to pay for it this time and direct them to financial aid options they can turn to in the future.

If they won't return the textbook and don't show any remorse for their actions, then I will tell the classmate that I will inform the student library of what I saw."

3 | Practice Timed Video Responses

The video response section of the Casper test is new for the 2022-2023 application cycle. It is critical that you take time to practice within this format and replicate the circumstances of the real test as closely as possible.

Practice answering sample questions in front of a computer camera within the one-minute time limit. Record yourself and review your progress. How do you sound? How do you look? Are you enunciating each word and speaking clearly? Are you speaking too fast or too slow? Were you able to fully answer the question before your one minute expired?

The video response section is a whirlwind experience; you'll receive one question after another, with only seconds in between each to get ready for the next one. It's crucial that you prepare for this section to ensure you don't freeze on test day. You will not be able to re-record any of your answers, and your ability to adapt in the face of mistakes, calm your nerves, and move forward is critical to your success.
